$mysql_username=' ';
$mysql_password=' ';
$mysql_db='';

$mysql_host=' '; = the root of your host's webserver - usually "localhost"
$mysql_username=' '; username that you use to access your server
$mysql_password=' '; password that you use to access your server
$mysql_db=''; name of database you are trying to access
