I am looking for (Telli) someone to create a script for me... for which I will pay.

I would like a User Form for users to enter some data (about 10-12 fields or so)....and have the data entered into a seperate mySql database. I would also like a clean, easy way to dump the final data so that it is easier to read than the normal data dumps and backups that Nuke creates.

FYI - This would be an annual community pool registration form so users can input their name, address, phone number, all family members names and ages, etc.... and a few other lines of data and maybe a few radio buttons or options to select.

Anyone wanna quote me on something like this ... or offer some other ideas?
