Here's part of the problem:
<img src="file:///C|/Documents%20and%20Settings/Steve%20Walsh/Desktop/images/spacer.gif" width="1" height="20" alt="">

You need to change the image path to the image on your webhost. That path is trying to pull the image from your computer. Change it everywhere you see that path.

Here's another part of your problem:
<object classid="clsid:D27CDB6E-AE6D-11cf-96B8-444553540000" codebase="http://download.macromedia.com/pub/shockwave/cabs/flash/swflash.cab#version=5,0,0,0" width="100" height="26">
          <param name="movie" value="djs">
          <param name="quality" value="high">
          <param name="bgcolor" value="#67A34D">
          <embed src="djs" quality="high" pluginspage="http://www.macromedia.com/shockwave/download/index.cgi?P1_Prod_Version=ShockwaveFlash" type="application/x-shockwave-flash" width="100" height="26" bgcolor="#67A34D"></embed>
        </object>

You forgot the file extension .swf.

But the biggest part of your problem is that you let ImageReady and Dreamweaver do the work for you. The table code that ImageReady spits out takes into account all the extra image slices and spacer gifs, so you have a whole lot of unnecessary colspans and rowspans in your table code. Your best bet is to scrap that code and start over and code it by hand, putting together only the image slices you need and getting rid of all that extra garbage.

Also, when you're slicing your .psd, you might want to edit each slice and give it a name instead of letting ImageReady give it a generic name. It'll be a thousand times easier to put the page together if you do.
