You have to find one you like then use the Web Tools on this site to change it from html to php. Then just include it into either your my_header.php for the whole site or in one of the files for the theme if it is just for a theme.

Ok erase all that stuff and just use this js script call it like you did the others. Open the js script and change it to say what you want.
