Form help

I have created a new module based on the 'Feedback' module that comes with Nuke.

It is an application form. The form displays great but when i hit the submit button it comes up with an error 'Sorry no such file exists'

Here is the form code: <FORM METHOD=\"post\" ACTION=\"modules.php?name=$module_name\">

I've had it display $module_name to see what file it is looking for and it is the correct module name.

Here is the submit code: <INPUT TYPE=\"submit\" NAME=\"submit\" VALUE=\""._SEND."\">

Any suggestions? do i need to post more code for help?

not sure what i did but it works now. thanks to anyone that took the time to read and try and help.
